Image 1 – This is image of IOB bank life certificate in this you have to enter your Name ( pensioner name), Your payment number and IOB bank account number. This form is checked and signed by bank official and in case if you are not able to visit branch you you get it attested by any 1st class officer and send it to near by IOB bank branch. ( In case of 1 class officer authorization pensioner need not to present physically before bank official for submitting pensioner certificate.
Image 2 – This is employment/ re- employment certificate of Indian overseas bank. You have to tick applicable filled i.e. whether employed or not as per your eligibility and at the bottom of this form you have to do your full signature along with pension payment number, place and date on which you are submitting this form.
Image 3 – This form is for family pensioners only and self pensioners need not to submit this form. Family pensioner have to sign this form along with pension payment number, date and place.
